Routing Form

My boss wants me to create a routing form. Attach it to some magazines and
have each sales person sign it and make comments and then bring the magazines
back to him. He called it a "routing form" and referenced tabs etc. Does
anyone know what he's talking about?

Sure. Big lumpy organizations like the Navy still use
these paper slips. The Army too. Here is a link to the
