Is this message them 2 times in your database with 2 times the same
attachment?

Yes, but you can delete your copy of the reply if you want.

In the 'outgoing' tab of the rules dialog, you could set up a rule to either
'remove attachments' or delete the whole message from all sent mail after it
has gone.

No. If a received message has an attachment, your reply to that message
does not. m.

Attachments are created only when you forward or redirect a message, I
believe; in that case, Barry's suggestions apply.
