Shadowed form field check boxes

A while back I read in this Discussion Group (and copied for future
reference) how to make shadowed form field check boxes instead of the regular
ones.

HOWEVER, I can't find it and for some reason, I couldn't retrieve it from
the search engines. I really need to know and would appreciate your help.

This time I'll make sure to put the answer in a secure place.
