Many people have looked for a template but they just don't exist. Each
company develops their own look. I always suggest trying to find something
online and then adapt to suit your needs.

Microsoft office templates I typed in Standard Operation Procedures and got
at least 4 pages of results. These might not meet your specific needs but
could be customized. I found using excel step in process description touch
points and needed materials for/from that touch point. We later added to
that adding expected timeline to create baseline against actual time to
process.
Another good way to get started is to create a graphic representation of
every step that needs to be followed and then put text to that. Hopes this
helps
