submit button driving me crazy

please help this rookie

this page works sometimes and not others
i have tried many things but still cannot get the submit button to function
all i want it to do is allow the customer to fill in the form and email the
results to my cousin (e-mail address removed)
currently i have it set to email me but nothing happens
i tried confirmation page, form changes, email changes, etc.

i would appreciate any help
