Survey anaylsis and report generation

Lost Medical Instructor

I am in search of a template that I can use to enter data from a survey and
track average scores of questions and number of respondants . I also need to
be able to generate a report that can be printed quarterly, biannually, and
yearly that has total number of respondants for that section and average
scores of questions. Are there any templates for this?

I found a template on the microsoft office website for the paper survey.
Now I need a way to analysis it.

nothing there works for what i am looking for.... thats why I did the post
here. any other place to look?
