Could anyone tell me if there is a template for a survey whicih includes checkboxes and options for respondents to comment. Thanks in advance

It's a bit like asking if there's a template for a novel ... just way too
general. Forms provide checkboxes and options. What are you looking for that
you hope a template will save time with? There are templates that offer
'Type heading here' kind of assistance, but that's really no help at all.
You still have to type the heading.
