Teacher's Lesson Plan Template

I need a good public school teacher's lesson plan template. I am using the
trial version of Office 2003. I need one for a substitute teacher, and for a
regular classroom grade school teacher(4th, 5th, 6th grades) and one for a
general music teacher K-6. An example of anyone of the three will help me get
started.
