Template for Sign out/destionation/sign in

I have a recovery Program and I need a sign out Destination & sign back in
sheetso when the clients leave I know when they left where they are going how
long they will be gone and when they will be back.

The Probation Officers want this and I need to come up with one,

There are various sign out templates available on Microsoft's site. (I know
this because I was looking for something I could modify for equipment sign
out this afternoon.) Obviously you are not going to find exactly what you
want (I knew I wasn't going to get lucky) but you can modify the templates
to suit your needs. In my case, I had a question for the boss and since he
left early, my task has been postponed until Monday. I may just do a simple
Excel spreadsheet and be done with it. Fancy fonts and formatting really
aren't necessary.
