Template to evaluate a training program?

The training website in Office Online has a Demo titled "Develop an Employee
Training Program" has a template "Training and Professional Development
Presentation Evaluation". I am unable to locate on the Office Online
website. Does anyone know where I might be able to locate this?

This would perfectly to evaluate our training program.
