Turn a form into a report

You'll need to manually set up the report. However, you
can copy/paste the controls from the form onto the report
to save time with layout. If your report uses the
same 'RecordSource' as your form you won't even have to
change the 'ControlSources'. The main difficulty with the
conversion is grouping. Reports are much more flexible
than forms. Good luck.
