What could cause multiple attachments?

BACKGROUND: We have an MS Access application that accepts
employee recognition nominations from an on-line form.
We review the nominations for eligibility then click a
button that sends a note to the awardee and their manager
with an attachment of the original nomination as an rtf
file. This is done with a VB procedure.

PROBLEM: On a few occations, it sends MULTIPLE
attachments. So one receipient is recieving their own
nomination PLUS someone else's. This is rare, but does
upset people when it happens.

QUESTION: What are possibile causes?

BACKGROUND: We here in the newsgroups love solving problems
as to why someone's code is not working correctly. It's almost an obsession
to some.

PROBLEM: On the occasions where the poster doesn't include the code,
we find it very difficult to help, even though I have seen instances where
a liitle telepathy was at work.

Question: Can you show us the relevant code?

Seriously though, post your code and we'll be glad to help out!
Altough it may be hard to debug an intermittent problem.
