what was used to make template in Safety Policy Violation Report

I'm trying to make a document/form where I can put a company header, a few
lines explaining what the form is then go into fields that are preset and
only need to fill in required information...Like the Safety Policy Violation
Report in the Office online help page. The fields need to be locked. The
information cannot force the fields to get larger. Pressing enter should
drop to next line if space permits, if not there would be space provided at
end of document.
