Word data query / import ?? ((HELP))

Hello



I am wanting to have a word doc that has a popup box to ask for
initials, this will then query a db (sql) and fill in the person name
and address etc.



Problem is I dont know where to start....... can anyone point me in the
right direction ??

experience do you have?
