Cache Mode Email sent delay

We have a very odd issue that has been getting more prevelant and
affecting
more and more people as time passes. Here are the facts: We are
running a
single Exchange Server 2003 Enterprise version SP2 (all patched up),
we have
several stores, clients are on Outlook 2003 SP2 (all patched up) in
cache
mode. Several people have been complaing that their mail is being
delayed
(even when sending from internal to ineternal). I have been able to
track a
lot of facts down and have come up with a specific issue ... Randomly
a user
sends an email from their desktop and it dissapears untill they log
into
webmail, terminal server, or some non cached outlook client. As soon
as
they log into a non cached client their mail sends. This is very odd
becuase the user can immedatly resend the message that has not sent
and it
will send on their cached client but then several days later they log
into
terminal server and the email they tried to send the first time will
send.
It seems the email is getting a time stamp of the client when they
tried to
send the message origonally but then the exchange server email
tracking
shows the message accepted and delivered to the reciepant mailbox when
the
mail actually sent. I have searched around for more techinal info on
exactly how the outlook client works- it appers that the client stamps
the
message and puts it in the sent folder on the clients cached machine
but
exchange does not see it as needing to send untill they log into a
non
cached email client. When I run ostscan on the persons mailbox the
below
sync error is what I get and this happens to be the message that was
the
issue. I tunred on logging in outlook but not sure what is causing
this
issue.

On another note I had a user send me a sync log error that shows 2
items
trying to change the read state on a mail item, anyone know what could
cause
this, she has a blackberry and running mcafee virus scan.
