Editing contact #s crashes Outlook 2003

When editing or adding a contact telephone number, Outlook 2003 crashes. The user has a desktop and laptop and this error only occurs on his desktop. Please note this also occurs in safe mode when all add-ins are disabled. The following information is listed within the error message:
