2 questions: weird source, 2 sources

1st question: i have is that my merge list is structured
as such:

ID date code description

where ID and date are not unique. there are no set # of
records per ID or ID-date.

I want each newly merged document to list all of the
dates, codes, and description, merged by ID. is this
possible?

2nd question: the above document is actually page 3 of a
long form, in which pages 1, 2, and 4-10 are populated by
a simpler query where each ID (same ID) is unique. can i
do this all as one document? would i have to
insert "page3.doc" into "restofpages.doc" somehow?
