A good tutorial on creating forms

Where can I find a good tutorial on creating forms? I have tried Word help,
but it lacks the depth and detail I am looking for. I want to create complex
forms for a standard format training manual. The various table-like data
entry boxes must be fixed on each page, but the content will vary with each
topic.

Assuming that what you are talking about is what Word calls an "online

Chapman's series of articles. You may also want to look at
http://www.word.mvps.org/FAQs/TblsFldsFms/LinesInForms.htm. Chapman's series
are the best tutorials I know of and take you from basic forms to userforms.
