Check with the trade show to find out the requirements that
are needed. Most trade shows have their own forms to
request space, tables, etc.

So download one for one of the trade shows you will be going to and save it
for future use. Don't be surprised though if a future show doesn't accept
what you send them.
I have had to do this for many shows, booking space in their exhibit hall,
and they always have their specific forms and contracts.

You can make your form as generic as you want.
I guess I should say I would like a "Wish List of Trade Shows" form that our
Reps could fill out and send in to us for review. Very Generic. As they see a
show that they would interested they fill out this form and send it in to
us. We then could go to the web, gather everything that we need and decide
whether it would be lucrative for our company or not. Cutting the middle man
(and mail) Unfortunatley we have to do it like this because our reps need
everything simplified for them.
