A Template On Authorization For Automatic Billing

I need a template for authorization of automatic credit/ debit card billing ,
can anyone help me find one? I know that when we set up automatic payments
with our utilities we sign a for within the same silmilarities, and I will
settle for that template if someone can tell me where to find it.
