As Greg says, one approach would be an underlined form field. See

If you don't want to create a protected form, you could use a MacroButton
NoMacro field as described in
http://www.gmayor.dsl.pipex.com/Macrobutton.htm. If you use the field
syntax { MacroButton NoMacro ____________________________ } and underline
the field, then there will be a line in the document to begin with (you
can make it any length you like), and when you type on it, the typing,
which will be underlined, will overwrite the field (assuming that "Typing
replaces selection" is enabled on the Edit tab of Tools | Options).
