Access 2k Survey

I need some assistance building an access 2k survey. I made a tblQuest and
since my answers are yes/no I plan to use an Option group. Then I reate a
tblQuestAns, that stores the user response(for example this table will store
the userID, QuesNum, and QuesAns). The problem I have is how do I fill this
tblQuestAns? Any assistance would be greatly appreciated even a sample,
speculation is also appreciated.
