Access add-in menu doesn't show when using File/Open vs. double-clicking on the .mdb file

I have a managed add-in (COM; not VSTO) with a shim DLL, which is
successfully loaded in MS Access *if* I double-click on the .mdb file to
launch Access.

That is, the add-in creates a custom menu, which shows up below the Access
toolbar if I launch it via double-clicking on the .mdb file in Explorer.

But if I launch Access via Start, All Programs, Microsoft Office, Microsoft
Office Access 2003 (or 2007), then custom menu doesn't show up when there's
no database opened, *AND* it also doesn't show up later if I then do
File/Open to open the .mdb file.

Also, if I click on the 'COM add-ins' command and try to 'Add' my shim DLL
via clicking the 'Add' button, it neither shows my add-in, nor is there any
error.

I've even manually registered the dll successfully, and it's all been signed
and the macro security settings are set to 'low'...

Any ideas why this isn't working when the file is opened in this way?
