Is it possible to access a Visio drawing in a .doc file to make change
without having the drawing? Is it possible to convert the "Visio picture"?

depends on how it was 'inserted' into the word doc. If ole then the link to
the drawing should be there, if it was inserted as a bitmap object then no.

Further to Al's comment - if OLE (for example Copy/Paste the drawing into
Word, or in Word Insert | Object | Microsoft Office Visio Drawing), the
recipient's machine would need to have Visio to be able to open the drawing
in Visio for editing.
