Accident/Incident Investigation Form

Does anyone have a form used to collect data for work-related incidents and
vehicle accidents that you would share? I am using Microsoft Vista OS and
2007 Microsoft programs.

Visio has some Crime Scene Investigation and (Vehicle) accident templates
that could be of use to you. Do a Google search on "Visio" and "Crime
Scene"...
