Add New Record to a Table with a button

Hello friends

I have a table SUPPLIERS that consist of ID, IDCITY, SUPPLIER and another
called table CITIES which it consists of ID, NAME....

..... and wanted to make a form with a button to add new suppliers.

before adding a registry, a city would be due to choose and just soon to be
able to add a new SUPPLIER in the city that corresponds in the table
SUPPLIERS to him. I hope they understand to me and they consider that I am
