Add ole image to Form/Report

I am trying to add an image to an OLE field in a
form/report. Everytime I do it using the insert->object
menu path. The only thing that shows up in the field is
an icon for the GIF or JPG that I am inserting and not the
picture. The "display as icon" box is unchecked. I'm not
sure what other steps there are but it is really
confusing. Especially since everything I am reading says
that is how to do it.

Any ideas?
