Added a form to web site using publisher and not working? Help

I have created a web site using Publisher and a templet web form was created
from publisher. After the web site was published using http protocol, i tried
to submit the form which I have set to send to my e-mail, but get an error
page saying the page not found.
Any clue what is wrong. FYI, Front Page Server Extension has been installed
correctly and I have checked with AT&T twice. I am looking for someone to
walk me through this seeing I am new at this. Thanks...

We need more information.

What is the URL to the page where you have the form?
Which version of Publisher?
Are you running XP or Vista...IE6 or IE7?

You might change the email address in the submit properties from the one
your are using to one that uses your domain name.

I hope you don't expect people to put their credit card info on an insecure
email do you?

There's a problem with FPSE on that page, the extensions may be broken or
not configured correctly to handle the forms.

I changed the e-mail but it still don't work??? No the credit card info it
was in the form and I plan to remove it once I get the form to work and then
challange the task in setting that up.

I tried and tested the same default form, and when I look at the source code
on the page I see a difference between your page and mine. Here is a snippet
from my form code:

b-email-subject-from-field="FALSE" startspan--><input TYPE="hidden"
NAME="VTI-GROUP" VALUE="0"><!--webbot bot="SaveResults" i-checksum="43374"
endspan -->

And Yours:

b-email-subject-from-field="FALSE" startspan--><strong>[FrontPage Save
Results Component]</strong><!--webbot bot="SaveResults" i-checksum="6561"
endspan -->

Perhaps Rob or someone else will understand the difference, and perhaps you
can check with support at your ISP and ask them the difference. I would try
uninstalling and then reinstalling the FPSE. If that didn't work, I would
try deleting your slightly edited form, and insert and test a new version,
and not make any changes to that form initially and see if you can get it to
work.

Sorry I can't offer a cut and dry solution. FPSE are finicky beasts and the
industry is moving away from them. Most ISPs offer an alternative form
program that you can use to power your form. Look in Support to see what
they have. You might change the submit button properties to point at that
program instead of the FPSE. What is the name of your host?
