Adding Contact Selector to Custom Control

I'm trying to add the Contact Selector to my Custom Controls, as shown here:

, and when I select it in the ActiveX Control list, and hit Next, I receive
this error:
CoCreateInstance failed to create the control.
- The control may not be properly installed or registered.
- The control may have missing dependencies.
- Only controls implemented as InProc servers (.DLL or .OCX) are supported.

-Any ideas?

you may find a ContactPickerIntl.dll in your clients "C:\Program
Files\Microsoft Office\Office12\1031".
Try copying this to "C:\Program Files\Microsoft Office\Office12"
