adding/including a fillable form in presentation, then send data to mailbox

I'd like to include a Word fillable form in a presentation. Then I'd
like that form data to get sent to a mailbox with one click ie "Submit"
or "send". Is there a pretty simplistic way to do this?

I have already created the Word template fillable form. Thanks for any
response!

Thanks Michael!
I was afraid of that. I've been scouring the net investigating CGI,
html, action buttons, macros, you name it. But it seems I should be
able to at least link to the Word form. All files necessary would
reside on the presenters' Laptop. Any ideas out there?

Ha ha! Zilch...but I'm old and grew up on DOS from the beginning. I can
learn enough of anything to solve a problem. I'd try, love to learn new
technology and put it into practice. There's gotta be a way....

In addition to what Austin has suggested. You can insert the word document
as an object which might work. The only thing would be to have the Word
document in the same folder as the PowerPoint presentation when you insert
it and the recipient would have to put the PowerPoint presentation into the
same folder as the Word document when they received it. I would be more
inclined to go with Austin's method. What I suggested is just a WAG.

Partial Success -
I DID end up using the Action Button method in PPT. It's OK, because it
is the last step on the last slide of the presentation. This will work
as it is being presented by only one person and mailed to only one
person, although it could be mailed to others.
In the Word form template, I graphically mimicked the PPT slides in
design and format.Then I linked the two, using the Action Button and
the transition is pretty smooth. The next step is using either
Mailmerge from the Word template or the routing feature of Outlook,
just can't remember which worked. I did successfully got the form
filled out and mailed to myself. With it's formatting and all.
I documented it, as if I don't use it, I lose it.
A BIG thanks to you both. You saved me time not trying to accomplish
the impossible.
