Adding states to a form

I created a form, but would like the address section to have a drop down of
states to choose from. Can anyone help?

I did a search on google for "drop down list US States" and found this
http://www.internet-promoter.com/code-us-states-drop-list.htm
Just copy and paste the code into your form in code veiw where you want the
drop down to be and change the form field ID if you need to
