Adding the Calendar Drop-down to a custom form

We've created a custom New Task form which contains some additional date
fields. How do I add the calendar drop down to those new date fields?

Outlook itself does not include any date picker control (which, by the way,
is one reason why the first page of the appointment, journal, and task forms
