AddIns repeated in Tools, templates and AddIns

We are finding multiple copies of the DOCS open add-in
(wordin2000) in tools templates and addins. we have
searched far and wide on the machine and there is only one
copy of this addin. when we delete the addin Word still
indicates that there are 2 copies of this addin. one
checked, the other unchecked. Word behaves erratically
and crashes when this problem is occurring.

Any clues?
