Advanced Filter

I'm trying to use an advanced filter in Excel 2004. In the example below are
the three ranges: first the criteria range, second the data, third the
extract range. Originally this "test" worked correctly -- the only record
appearing in the extract range was no. 4, which matches "Miami" in the City
field. However, you can see the result that this is now producing. Can
anyone figure out what's gone wrong? The three ranges are defined
correctly, I've rechecked this several times.

The only way I can replicate this is if the criteria range is incorrect
- say, only the header row, or more than 2 rows.
