After creating form

After creating form I sent to my self by using "new mail message". But it
reaches to me not like a form. Why? How to solve this?

It's an option on the Properties tab so that you include all the data in the
form - otherwise, if the recipient doesn't have the form installed, they
won't see the form.

You should also send the message using RTF format - the forms use TNEF
encoding.

1. if you create a form and send it to someone who doesn't have the form on
their computer, their outlook will display it in the standard outlook for
for that item type.

2. click on the recipients address (if contact opens, click on the email
address in the contact) and choose 'always send rich text'

Dear Sir,
I have asked you in different way. Still I could not do this.
Once you can educate me in this regard I will be very happy in wonderful way.
Because for last 2 years I am struggling to get this information.
I dont know why not working this way. Please help me help me.
