Synapse Syndrome

Since I have started using Outlook 2007 (with Vista) I have never seen the
alert popup come up when I receive email.

In Tools > Options > Preferences > Email options > Advanced Email Options >
Desktop Alert Settings

...i can set the settings and see a preview, but I never see this alert.

How do I configure Outlook to display this alert?
