Appointments falling off

Below are some comments from Administrative Assistants
within my organinzation. This is, and has been, happening
to several of them over the past few months. I'm just now
finding out about it:

"I have learned one thing (happened again in the past 24
hours).when I have a regularly scheduled recurring
appointment, and I change one instance to a different day
and/or time, I accept the appointment, but it drops off.
That seems to be the biggest issue."

"It seems when I have a regular recurring appointment on
my executives calendar, and I change one instance of that
appointment, the one changed instance "drops off" my
executives calendar (but does not seem to drop off the
attendees calendar)."

"There have also been a few other "drop offs" that do not
fall into this category, but I have not figured out a
pattern with those."

The following are articles I've seen and tried without
success. Does anyone else have any other ideas? Any help
will be appreciated.

What version of Outlook?
Do you connect to an Exchange server? If so, what version?
Can you see the appointments in OWA?
Are your clients setup with CACHE mode? This is not suggested
for DELEGATE type of situations. (Calendar Cache)
