Are there bugs with Outlook follow-up flags?

Outlook 2003

We have constant issues with follow-up flags. Either the color doesn't show
up properly or they don't appear as a follow-up reminder. It has happened
with different users and each has different issues. It just appears to be a
problem with flags in general.

Does anyone have any information regarding this?
