Authorisation Form

Hi

I can design a form that is submitted to a database and an email. I would
like to take it one step further so that when the new user form is submitted
to me I then email another linked form to the person that has to authorise
it.

But how do I get the authorisation form to pick up the name of the person
that is requesting the authorisation. So that the person authorising
doesn't have to type anything in.

Hope that all makes sense!

Any ideas would be great
