auto-send not working

Microsoft Office Outlook 2003 ( 11.8217.8202 ) SP3

I have configured all the options I know of to ensure sends occur
immediately.

Tools > Options > Mail Setup > Send/Receive > Send immediately when
connected

Tools > Options > Mail Setup > Send/Receive > Send/Receive Groups > Schedule
an automatic send/receive every 5 minutes


And yet emails sit in my outbox until I manually Send/Receive ...

No, I do not.

Could my problems be related to Exchange Server config ?

Do you have Outlook set to work offline? File->Work Offline?
