Opinion...is it necessary to place a "Back" button on each screen? Common sense would tell me that I can simply hit the back button at the top of the screen. But do you think people generally hit "home" or a "back" button?

It's not necessary but a lot of designers do it.

There are situations where they are appropriate, and even beneficial.
Usually if the pages are part of a multipage process such as a form or a
long document of some type.

I would tend to agree with you that they can be overkill. I'd say that in
most cases, a good navigation element, such as a toolbar, makes most back
buttons redundant.

Your common sense is right on. While some web sites have such navigation
built into the HTML, the reason that the browsers have a "Back" button is
that it is quite common for a user to want to navigate back to something
previous. Therefore, it is somewhat superfluous to include an HTML "Back"
button. On the other hand, it does no harm.
