BCM Detail window says "BCM was unable to initialize this form"

When I try to open a contact or account detail the above message is written
in gray across the detail window. I have had it working but had some trouble
with another program and BCM became disabled in the repair process.

Your Forms might have got corrupted. You can go to
C:\Users\"USER NAME"\AppData\Local\Microsoft\FORMS\ and delete FRMCACHE.DAT.
Restart oulook. that should fix your form corruption.
