Beginning and Ending Dates in a Dialog Box

I hope I can make what I am asking make sense. I have a dialog box with a
combo box set up to allow users to pick a location and then run a report.
Picking the location seems OK.

I need to also allow the users to enter a range of dates, so I left the
BETWEEN [enter start date:] AND [enter end date:] in the query. This is
causing the date questions to be asked twice and I cannot determine why. If
I need to supply more information, please let me know.
