Birth Certificate template for twin boys?

to show:- hospital of birth,date, hour,weights,names,mother's name,father's
name,doctor's name,midwives names.

(border of certificate to show small toys,balloons,kites,trains,etc.)

Yeah, I was thinking that, too. But maybe for a home birth, you might want
to use something you created and have the midwife (or whomever) sign off on
it. Probably they file an official one, too....

I try not to question motivation too much. :)

Those you get from the hospital are for keepsakes and aren't official.
Usually they are a little plain so many will create their own so they can be
framed.

Really? (Can you tell I opted out of parenthood?) My gradiose plans are to
make birth samplers for all of my nieces, grandniece, and grandnephews. As
if it isn't bad enough that I'm several years behind, the two oldest nieces
are both due before then end of the year.

Yes, really. You need to obtain an official birth certificate for school,
daycare, whatever, at the local county building in the county the baby was
born in. (usually that's where). And, of course, there's a fee involved.
(btw, you want the keepsake birth certificate before birth so the
footprints, fingerprints and be added at birth. ;-) )

I can't believe all you people! all i was after wasa template on how to do a
type of "birth certifcate" for a "keepsake" . I did not want a
birthcertificate. I know where to get this!

(All the more reason to follow Echo's motto: I try not to question

another question in this thread about the "officialness" of the birth
certificates you get from the hospital. I knew you were only after keepsake.

I don't have children so I've never heard of a keepsake birth certificate.
Never even considered anything like that. Like I said, I'm a needleworker so
I'm more familiar with desiging a birth sampler for that.
