Business Contact Manager w/ Outlook 2003

After installing Business Contact Manager and it's update on an installation
of Outlook 2003 with Service Pack 1, I get the error:

"Error loading form. Some of the needed controls may not be registered." I
cannot find any information regarding a fix anywhere. Any ideas?
