Cached Exchange Mode -- Outbox items are not being sent out

In trying to improve response time when previewing Inbox items, I turned on
the Cached Exchange Mode. Now items in my Outbox are not being sent out,
even when I use Tools -> Send/Receive -> Send/Receive All.

The dialog box indicates all completed without issue, my status bar
indicates "Connected" with "Download Full Items" selected, and "All folders
are up to date.", but the items are still sitting in my Outbox.
