Can I create an Access DB as a survey builder tool?

I have created a table of various questions that could be asked on a training
evaluation form. I want to create some format that allows users to select
several questions from the list and have it dump into a report format or Word
document. Plus, I want it to automatically account for different question
types (i.e. Yes/No, Likert scale, open-ended).

Is this possible in Access?

Thanks for the link. This looks like it a good example for me to follow to
set up my own system. I just didn't have any idea where to start.

Ok. I just want to build the survey in Access and have it print onto a paper
form - participants would not be at a computer to complete it and I don't
need it to tally responses.

Well, you can print your survey from "At Your Survey" and the applications
is not designed to have people take the survey on-line. Generally someone
puts in the responses after the fact. But a question: If you are not going
to tally the responses, why have a survey? Surely you want to see the
responses aggregated. That's the purpose of surveys.
