If you are using a payroll package then ask your vendor for some payroll
stationery.
If not, then just design your own!
All you need is Employee name, works number, month or week, pay for the
period, deductions, (ie tax etc) and net pay. You can include
year-to-date figures if you like.
What's difficult about that?
