Can the Org chart redraw when database adds/deletes employees?

I want to attach the Org chart to the database in such a way that the Org
chart sees who each employee reports to and redraws the org chart to reflect.
I want to be able to delete / add employees and the org chart reflect the
changes immediately.

It might be possible if your database engine has any events available when
user changed data. Otherwise, the key might be how can Visio know when
external database is changed.
