Cannot create a header

We have rec'd documents where users could not create a header because one
does not exist.

I have:

...viewed in Normal or Print Preview and attempted to double-click where the
header should be ..nothing
...accessed the footer and tried to use the toolbar button to "switch to
header" ..nothing
...checked the font to make sure it was not on white within the footer style
...checked the page layout margins
